Our chief registered nurse (RN), who is credentialed with a
Bachelor of Science in Nursing (BSN) and two (2) year Psychiatric
Nurse Diploma, is certified as a Geriatric Care Manager and will perform
a complementary assessment with a holistic care objective. We believe that
the individual and family are the center of the Care Model, and our goal is to facilitate the realization of that goal for each client and family. We strive to match every client with the appropriate caregiver so that our clients feel they are “with family.”
In collaboration with the client and family, we develop an individual care plan including services as needed such as
  • Personal care (bathing)
  • Grooming
  • Assistance with transfers
  • Turning
  • Use of transfer lifts
  • Range of motion exercises
  • Assistance with mobility
  • Meal preparation to include special diets
  • Toileting
  • Incontinence care
  • Medication reminder and/or nurse to set up weekly medication planner
  • Transportation/escort services
  • Attending to pets and plants
  • Running errands
  • Light housekeeping
